FREEDOM OF INFORMATION ACT 2000 – REQUEST FOR INFORMATION REFERENCE FOI/6456

Thank you for your request for information relating to Adult Social Care Contacts.

We are now able to provide the following information:

I am writing to you under the Freedom of Information Act 2000 to request the following information relating to Adult Social Care services within your authority:

  1. The name, job title, and contact details (including direct email address and direct telephone number if available) for the current Head of Service(or equivalent lead officer) responsible for:
    • Learning Disabilities
    • Mental Health
    • Reablement Services
    • Older Adults Services
    • Assistive Technology
    • If separate individuals cover these areas, please provide the above information for each relevant postholder. If any of this information is already publicly available, please direct me to the appropriate source. I understand that under the Act I am entitled to a response within 20 working days. Should you require any clarification of this request, please do not hesitate to contact me.
